Do you have any problems like this?
We accept inquiries from patients, families, care managers, hospital counselors, and facility staff.
Going to the hospital has become difficult.
We provide medical treatment at home or at a facility for people who have difficulty going to the outpatient clinic due to weakened legs, dementia, bedriddenness, burden of nursing care, etc.
I’m worried about my life after being discharged from the hospital.
We work with hospitals, visiting nurses, and pharmacies to support you in managing your physical condition after discharge from the hospital, adjusting medications, providing medical treatment, and providing advice in the event of sudden changes.
I want to spend my last moments at home or at a facility.
We provide support for home care and end-of-life care that alleviates pain and breathing difficulties, and is tailored to the wishes of the patient and their family.
Persons eligible for home visit medical treatment
We focus not only on the name of the disease, but also on whether it is difficult to go to the hospital and whether medical care is required at home or at a facility.
We provide palliative care and end-of-life care for pain, nausea, shortness of breath, anxiety, etc.
Continuously check for home oxygen, shortness of breath, edema, weight changes, and changes in respiratory status.
We provide support including medication management, decreased food intake, fever, falls, nursing care burden, etc.
Please consult us regarding gastrostomy, urinary catheters, bedsores, intravenous drips, injections, pain management, etc.
We address issues including breathing, nutrition, pain, decision support, and disaster preparedness.
We support your recuperation life in collaboration with care managers, visiting nurses, pharmacies, and facilities.
In times of sudden pain or anxiety, bring medical care to the patient.
A hospice car is not a vehicle that transports patients, but an emergency vehicle that delivers doctors, nurses, emergency medical technicians, and medical supplies to patients.
We will be dispatched while checking the necessity and safety of cases such as severe pain, difficulty breathing, near end-of-life care, and continued home medical care in the event of a disaster or power outage.
From consultation to start of home visit treatment
To make it easy for first-time patients to understand, we have summarized the flow from consultation to the start of home visit treatment.
Please contact us by phone first.
Check the patient’s condition, current attending physician, care services, visit location, and urgency. Consultations are also available from family members, care managers, hospital counselors, and facility staff.
Check the information sheet, etc.
Check the current medical condition, prescription details, test results, nursing care status, discharge schedule, and visiting nursing/pharmacy status.
Confirmation of first visit, contract, and treatment policy
We will ask about the wishes of the patient and their family, and confirm the frequency of medical treatment, how to contact them in an emergency, their wishes for end-of-life care, and their partner.
We will start regular home visit medical treatment.
We visit patients regularly depending on their condition, and when necessary, we make house calls, dispatch a hospice car, and coordinate with visiting nurses, pharmacies, and hospitals.
Compatible area
We will respond mainly to Aira City, with a radius of approximately 16 km from our hospital. We will consult with you individually depending on the patient’s condition, visiting schedule, road conditions, etc.
Regarding costs
It varies depending on the burden ratio of medical insurance/nursing care insurance, number of visits, and treatment details. Before we start, let me explain it as clearly as possible.
In order not to stop home medical care even in the event of a disaster
Our hospital has a BCP in place, and depending on the patient’s severity, we will confirm the safety of the patient, make house calls, and dispatch a hospice car.
Prioritize priority patients
Mobacarnet’s classification of "most severe, severe, caution required, and normal response" will be linked to safety confirmation and home visit priority in the event of a disaster.
Delivering medical supplies and equipment
We check and transport the resources necessary to continue home treatment, such as oxygen, suction, drugs, intravenous drips, and infection protection equipment.
Support through regional cooperation
We work together with visiting nurses, pharmacies, hospitals, fire departments, governments, and facilities to protect the safety of patients and their families.
Inquiries regarding home medical care
It’s okay even if you’re not sure if you need a home visit. Please tell us about the patient’s condition, discharge schedule, nursing care status, and any current concerns you may have.
